Rutgers Prep Athletics

All athletes must turn in an Annual Physical Exam Form (completed by a physican and a parent/guardian) to the school nurse before they are eligible to play an RPS sport.  This form is is turned in once per school year and is kept on file in the nurse's office.  If you would like the athletic trainer to have access to the physical form please make a copy and turn it in with the Athletic Participation Form to the athletic trainer.  

All athletes must turn in an Athletic Participation Form (completed by a parent/guardian) to the athletic trainer prior to the start of each sport's season that they will be participating in.  This form must be dated no more than 60 days prior to the first practice of each sport's season.
